P289 Intestinal obstruction syndromes in cystic fibrosis

Meconium ileus (MI) presents at birth as small bowel obstruction. Distal Intestinal Obstruction Syndrome (DIOS) is a faecal obstruction of the ileocaecum after neonatal period. We undertook retrospective review all patients our CF Centre to characterise spectrum intestinal syndromes. 404 patients, 36(9%) had presented with MI. All pancreatic insufficiency; 21(58%) were male; 26(72%) required surgery, 10(28%) resolved gastrografin enemas; 17(47%) developed DIOS over their subsequent life course; 19(53%) no mean 28.5(19–57) years. 63 (16%) –42 male (67%)-developed 118 episodes DIOS, requiring hospital admission; 39 single episode, 24 recurrent (2–12) episodes. 35(56%) F508del/F508del, 26(41%) F508del/other; 6 post-lung transplant. Gastrografin was used treat 109 episodes, via nasogastric tube in 77, gastrostomy and orally 26, laxatives 2 colonoscopies terminal ileum; parenteral nutrition during prolonged DIOS; 7 surgery for unresolved or complications. Other causes adhesions 12(3%), intussusception 5(1.2%), volvulus 2(0.5%). There 3 (0.75%) cases fibrosing colonopathy childhood, 3(0.75%), cancers C.difficile colitis 3(0.75%). acute abdominal pain cholecystitis 33(8%), renal calculi 27(7%), appendicitis 18(4.5%), pancreatitis 16(4%) ovarian conditions 8(2%). an unpredictable emergency, which must be distinguished from colonic constipation other problems. Pathology mimicking distal intestinal obstruction syndrome in cystic fibrosis.

Eight patients with cystic fibrosis had chronic abdominal pain and the other features of distal intestinal obstruction syndrome. Coexistent abdominal pathology was shown in six patients. Two had a small bowel volvulus, and the others had Crohn's disease, a small bowel fistula, appendix abscess, and an ovarian dermoid. Opiate abuse exacerbated symptoms in two other patients.
